2. Hazard Identification

3. Procedure

3.1 PREPARATION FOR SHUTDOWN

  1. Identify Energy Sources: Before beginning any maintenance or repair, identify all forms of hazardous energy (electrical, mechanical, hydraulic, pneumatic, thermal, gravity) connected to the equipment.
  2. Notify Affected Workers: Inform all operators and workers in the area that the equipment will be shut down and locked out. Explain the reason for the lockout and how long the equipment will be unavailable.

3.2 SHUTDOWN AND ISOLATION

  1. Normal Shutdown: Turn off the equipment using the normal stopping procedure (e.g., press the stop button, flip the toggle switch).
  2. Isolate Energy Sources: Locate and operate the energy isolating devices (e.g., disconnect switches, circuit breakers, valves) so that the equipment is completely isolated from its energy sources.
    • Do NOT use control circuit devices (like push buttons or selector switches) as the primary means of isolation.

3.3 APPLY LOCKOUT AND TAGOUT DEVICES

  1. Apply Personal Locks: Every worker involved in the maintenance must apply their own personal lock to the energy isolating device. If a device cannot accept multiple locks, use a lockout hasp.
  2. Apply Tags: Attach a tag to your lock indicating your name, the date, and the reason for the lockout (e.g., "Do Not Operate - Maintenance in Progress").

3.4 CONTROL STORED ENERGY

  1. Relieve, disconnect, or restrain all potentially hazardous stored or residual energy.
    • Bleed air from pneumatic lines.
    • Release hydraulic pressure.
    • Discharge capacitors.
    • Block or support elevated machine members that could fall due to gravity.

3.5 VERIFY ISOLATION (ZERO ENERGY STATE)

  1. Test the Equipment: Before starting work, ensure the area is clear of personnel, then attempt to turn on the equipment using the normal operating controls to verify it will not start.
  2. Return Controls to Off: After verifying the equipment will not start, return all operating controls to the "Off" or "Neutral" position. The equipment is now locked out and safe to work on.

3.6 PERFORMING THE WORK

  1. Proceed with the required maintenance, repair, or inspection.
  2. If the job is handed over to another shift, the oncoming worker must apply their lock before the outgoing worker removes theirs. The equipment must never be left without a lock if the work is incomplete.

3.7 REMOVAL OF LOCKOUT/TAGOUT

  1. Inspect the Work Area: Ensure that all tools, replacement parts, and debris have been removed from the machine. Ensure all guards and safety devices have been reinstalled.
  2. Check Personnel: Ensure all workers are safely positioned away from the equipment.
  3. Remove Locks and Tags: Each worker must remove their own personal lock and tag.
    • Note: A lock may only be removed by the person who applied it. If the person is unavailable, the supervisor must follow a strict, documented procedure to verify the worker is not on-site and it is safe to remove the lock.
  4. Restore Energy: Reconnect the energy sources by operating the isolating devices.
  5. Notify Affected Workers: Inform the operators and surrounding workers that the lockout has been removed and the equipment is ready for use.
